Add rhythm waveform support to ECG annotation pipeline

• Extend ECG beat/extracted data structures with rhythm flags and rhythm waveform buffers
• Propagate waveform vectors and rhythm state through feature extraction flow
• Integrate FWT unit test target into test build configuration
• Normalize FetchContent formatting and initialize rhythm state defaults
