test: add multi-lead ECG annotation and signal loading

• Add automatic ECG lead detection and mapping
• Support loading multiple WFDB annotation extensions
• Route raw and engineered samples by detected lead
• Store annotation data per annotation type
• Refactor sel103 test setup for multi-lead datasets
• Add annotation lookup support
