feat: add patient-specific ECG feature configuration and reset support

• Refactor EcgAnnotation configuration from legacy ANNHDR to EcgAnnotationConfigType
• Replace qrsANN/qrsNum with QrsAnn/QrsNum
• Add default ECG annotation configuration values directly to the config structure
• Add EcgAnnotation::Reset() to clear annotation state, CWT resources, buffers, counters, indices, and processing flags while preserving configuration
• Update ECG annotation processing to use m_AnnConfig consistently
• Add patient-type support for default, adult, pediatric, and neonatal profiles
• Add patient-specific RR, QRS, PQ, QT, P-wave, T-wave, and refractory-period limits
• Add SetPatientType() and GetPatientType() APIs
